We have an amazing opportunity for a Registered Nurse in our Home Stabilization Program. RN will plan and implement the assessment and coordination of care with clients, and their families from intake through referral planning and transition. Coordination of care is provided in collaboration with families, and community referral sources. Meeting with families to provide screening and assessment in determining appropriate referral plans and documenting progress in referral engagement. Help clients to understand the recommendations of their medical team, educate on various topics related to understand the importance of good nutrition, regular exercise, and good sleep hygiene and work with case managers and peer recovery specialists to support the client to improve their health and well-being.
