POSITION SUMMARY:
Assess, monitor and address the psychiatric, mental health, medical and care coordination needs of clients enrolled in Community Support Services Integrated Health Home.
EDUCATION AND TRAINING REQUIRED:
- RN required, Bachelors degree preferred. Current license to practice in Rhode Island
- Experience and/or interest in working with individuals with chronic psychiatric impairment and/or chronic medical conditions and/or substance addictions
- Current CPR/AED certification
- Desire and ability to work in a fast-paced cooperative, multi-disciplinary team environment
WORKING CONDITIONS:
- Must be able to read, write, speak and comprehend English.
- Must possess fine and gross motor skills required to handle individual and packaged medications, assess blood pressure and vital signs, administer IM injections
- Must be able to maintain concentration and organization and complete assigned tasks while working in close quarters with others.
- Must possess sound judgment under duress and communicate effectively with members of a variety of professional disciplines in complex circumstances.
- Must maintain a safe, insured, registered, working vehicle and be willing to transport clients therein
- Must possess or be willing to develop basic computer skills and utilize an electronic health record
- Must be willing and physically able to outreach clients in multiple community and residential settings, including via stairs, and including in inclement weather, and including in occasional incidence of residence infestation (personal protective gear and education is provided by employer)
- Must be willing to interact with individuals diagnosed with infectious diseases, adhering to all agency infection control guidelines and recommendations
