Health Navigator-Woonsocket, RI
Energetic Health Navigator needed for team-based support to assist adults in identifying their behavioral health and medical needs. Office- and community-based work schedule your day to include a variety of challenging and satisfying interactions with clients including Education of Medically Assisted Treatment (MAT), coordination of medical care with community providers, and initiate referrals to specialized care as needed.
Possess or willingness to complete Community Health Care Certification.
