Case Manager Tanguay Residential-Woonsocket, RI

We’re looking for an empathetic candidate who has experience working with individuals with mental health and substance abuse preferably in a residential setting.  This program is located in Woonsocket, RI.  As a member of our team, you will assist in the delivery of psychiatric, rehabilitation, and supportive services to adults living in a group home setting. Our program is designed to support residents in reaching their goals for living, learning and working in the community as independently as possible.  Duties will include: assessing and evaluating clients, providing support services, crisis intervention, medication monitoring and education as needed. Case Manager will be responsible for advocating for clients and coordinating with providers, educating clients, their families and significant others in regards to mental health and/or substance abuse issues and the impact on their lives.

Education/Licensing Requirements:  Associate degree in human services is a requirement; BA/BS is preferred.  Must have current CPR/AED & First Aid or willing to obtain.  Must have a valid driver's license, auto insurance and reliable vehicle.
