Safe Haven offers a set of supportive services for individuals who are unhoused and actively using substances, often with co-occurring mental health challenges.  The Integrated Health Home team is a multidisciplinary team offering psychiatry, nursing, case management, peer recovery support, hospital liaison, and housing support.  The primary focus is on care coordination and addressing the social determinants of health with the intention of improving health and well-being.  The case manager is integral to this goal, working one on one with clients to remove barriers to care.  Supports includes: connection to emergency shelter and housing, access to basic needs, primary care coordination, access to benefits/entitlements, navigation of legal issues, building improved natural supports and social environment. 

The approach used is person-centered and trauma informed.  This work is life-saving and life-changing. Each day is different.  Must be an effective communicator verbally and in writing.  Maintain good boundaries with clients.  Experienced with technology and able to use an Electronic Health Record.
