ACT Psych Rehab Specialist

POSITION SUMMARY:

The PSRS's role within the multidisciplinary team is to provide psychosocial rehabilitation services to psychiatrically disabled adults and their families, primarily in the community, to promote community integration and assist clients in achieving their recovery goals through case management, independent living and social skills training, social support and network enhancement, and access to leisure activities. Individual and group services are provided to develop, enhance, and retain skills and competencies in the areas of living, learning, working, and socializing. Through participation in the development and implementation of individualized Recovery Plans, PSRS will monitor and track the progress of clients on assigned caseloads.

EDUCATION AND TRAINING REQUIRED:

  • Bachelor’s degree required in psych rehab, psychology, social work, or other human services related field.
  • One year of experience in community mental health preferred.
  • Valid license, clean driving record, reliable vehicle and auto insurance required.
  • Employee will assume full responsibilities for the cost and efforts of maintaining any and all forms of licensure, certification and credentialing documentation as required in their job description or sited on any Action Form as a qualification for hiring, job promotion, or monetary increase. Subsequently, the employee will adhere to the credentialing requirements of all insurance/funding sources for which they qualify and provide Human Resources and/or the Credentialing Specialist with the initial required documentation (licenses, liability insurance policies, DEA numbers etc.) and all renewals of these documents.

OTHER SKILLS REQUIRED:

  • Demonstrates competency in clinical, technical and/or administrative skills, procedures, methods, and tools required to perform the job successfully.
  • Regularly available for work, reliable and on time for all things scheduled.
  • Ability to communicate effectively orally and written.
  • Competently and consistently completes work accurately and thoroughly and in a timely manner.
  • Self-starter works with minimal supervision, and as part of a multidisciplinary team, and constantly seek new/better ways to organize/complete tasks.

WORKING CONDITIONS:

  • 75% of services are delivered in the community; this includes home visits when appropriate.
  • Clients' transportation is required.
  • One evening shift per week (until 6:30 pm), one weekend shift per month, rotating holiday shifts.
