ACT Program Manager (Full Time)
Pay Range: 80k-90k
The Community Support and Recovery Services program is looking for a Program Manager for our Assertive Community Treatment (ACT) Team, a multidisciplinary team specializing in crisis stabilization, who works collaboratively with adults experiencing serious mental illness in order to empower them to make choices, attain life goals, build relationships, and take part in their communities. This is done in the most integrated and independent way possible by focusing on our clients' strengths and values, we provide culturally competent, flexible, coordinated, and accountable person-centered services.
Responsibilities & Duties:
- Direct the day-to-day operations of the ACT team. This includes coordinating treatment, rehabilitation, and support services in collaboration with the team to prevent crisis situations that may lead to psychiatric hospitalizations.
- Perform crisis intervention, staff supervision and consultation and utilize safe crisis management techniques during normal business hours.
- Review assessments and recovery plans for appropriate clinical content and oversee management of EHR records ensure compliance with various policies and requirements.
- Coordinate and supervise admissions and facilitate and monitor discharge planning of clients, ensuring we maintain our census targets.
- Establish effective relationships with clients, families, and other community providers and human service providers to ensure quality of care.
- Participate in the staff recruitment and selection process and develop and conduct staff orientation and training.
Qualification & Skills:
- LICSW/LMHC/LMFT/ LCDP/ RN
- 2-3 years solid community-based experience.
- Valid driver's license and reliable vehicle.
- Must have basic computer skills.
